All the feilds with asterisk symbol are mandatory.
All the Documents,Certificates and Marksheet should be in pdf format and their size should less than 1MB.
Application Form
Photo should be in jpg/png/jpeg format and photo size should less than 1MB.
Number of children on date of application if more than 2 then childern's date of birth in descending order and should be common separated.
For Example :- if No of Children 3 than date of birth will be like 15-06-2015,12-08-2018,07-05-2022.
Documents Required :- Identity Proof Certificate, Category Certificate if category is other than "General", Medical Council Registrations Cards.
Educational Qualification
No. of Attempts certificate should be merged, in pdf format and size should be less than 1MB.
